Job summary
We are looking for an experienced nurse to take up the post of Ward Manager to support the development of the following priorities & to support & drive a new model forward to deliver evidence-based practice to ensure we deliver the best patient outcomes.

-To provide outstanding step down & step-up rehabilitation

-To provide step-up care from the community, preventing admission to secondary care

-To provide palliative ward-based care for those at the end of their life

-To support the delivery of place-based care for people living in Mid & North Hampshire.

Alton Community Hospital works closely with GPs & local partners to ensure the population of Mid & North Hampshire have access to first class rehabilitation & step-up care.
