Hospital at Home (formerly Frailty Virtual Ward)

These posts will cover core working hours will essentially be daytime working. There will however be some requirement to work some weekend mornings and Bank Holidays. We are looking for individuals with a keen interest in developing a career in Elderly Medicine, General Practice or Palliative Care. All posts offered will be 12 months in length.

This role offers the opportunity to help shape the future development of the new and emerging Hospital at Home service within York and Hambleton, providing acute care to patients within their own home environment, supporting early hospital discharge and admission avoidance. There is expected to be further expansion into North Yorkshire.
