Job Description

**Pay:** $55.00 - $59.00 per hour based on experience
**Type:** Three-month contract
**Schedule:** Monday – Friday, 8AM – 4:30PM
**On Call:** Yes
**Coverage Areas:** West End, South Boston, Jamaica Plain, Back Bay, Roxbury, Beacon Hill, South End
**Primary Responsibilities:**
· Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and physical therapy needs. Consults with the physician in the development of the therapy plan of care.
