Responsibilities
- Conduct thorough assessments to determine the mental health needs of cancer survivors
- Administer evidence-based individualized and group survivorship therapy protocols
- Collaborate with MDs, NPs, and care navigators to address patients' mental health needs
- Support the implementation of an innovative cancer survivorship collaborative care program (CoCM)
- Develop comprehensive care plans and provide expertise in behavioral health interventions
- Deliver psychoeducation to survivors and their families using evidence-based content
