Staff Nurse : Home Healthcare Nurse

Assist clients and their caregivers to acheive seamless transition from various stages of the care journey (e.g. Hospital to Home) by providing customer centric and holistic support services.

• Conduct screening referrals and assessment of potential clients and their home environment;
• Develop and perform Individualised Care Plan (ICP) involving caregivers and/or family members (if possible);
• Monitor clients' conditions and conduct periodical review of clients' ICP and conditions;
• Conduct financial counselling and provide advice on government subsidies available;
• Collect payment/s and issue receipt/s for services rendered;
• Manage hotline (on rotation basis);
• Assist doctors during home visits;
• Develop Discharge Plan for clients who no longer require our services;
• Conduct health education and talks (to seniors and caregivers in the community);
