The Center for Health Equity & Community Wellness (CHECW), CHECWs work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well being of New Yorkers. The Office of the Chief Medical Officer (CMO Office) aims to bridge NYC's healthcare systems with the health department to expand and strengthen bidirectional communication and engagement around the emerging health needs and risks of NYC communities. These capabilities are achieved by coordinating and streamlining agency-wide provider communications, using content tailoring approaches by leveraging a health provider contact information database, and leverage community connections to increase agency reach to providers.
