Social Worker - West Community

Permanent Full‑time 1.0FTE – West Adult Community Mental Health Team

We are looking for an experienced New Zealand registered Allied Health professional to join our West Adult Community Mental Health Service. You will provide assessment and treatment interventions and collaborate with a multidisciplinary team to manage clients aged 18‑65 with a range of mental disorders.

Responsibilities

• Oversight and management of a varied case portfolio.
• Complete assessments of consumers and formulate interventions to meet individual needs in collaboration with the multidisciplinary team (MDT).
• Work closely with clients, their families/whānau, and carers to support the return of clients to primary health care after specialist input.

Qualifications

• Registration as a Social Worker NewZealand with an Annual Practicing Certificate.
