Job Description
Casework and Counselling
- Manage case referrals and active cases
- Conduct assessment, planning and intervention for cases as well as case conferences with partners.
- Participate in networking and case management meetings.
- Document and evaluate case outcomes.
- Develop and review appropriate intervention strategies.
- Ensure reports on cases are submitted in a timely manner.
- Refer clients to appropriate agencies for further intervention.
- Part of the Intake & Triage Team to conduct intake assessments for new intakes based on the practice standards of the Code of Social Work Practice and Practice Guide.
- Network with other organizations and stakeholders.
- Community Needs Assessment.
- Be part of the programme team for Group Work conducted at the FSC.
