Social Worker

Responsibilities

• Provide assessments and counseling services to individuals, families, and groups to meet the psychosocial needs of clients.
• Work with the interdisciplinary team to develop a treatment plan outlining therapeutic goals appropriate to the clients’ needs.
• Provide information, referrals, and client advocacy when needed.
• Facilitate client linkages with internal and external resources and effectively coordinate client support.
• Participate in community development, education on wellbeing and other issues based on the needs of the community.
• Take part in the evaluation and ongoing improvement of services and programs provided at the Health Centre.
• Help prepare, organize, and disseminate programs, as well as educational and preventive material relating to mental health.
