Social Worker - MSW- Care Management

Overview
The Social Worker at New London Hospital will provide assessment, counseling, information and referral, advocacy services, and other resource assistance to patients in both inpatient and outpatient settings. The Social Worker will act as a resource for the Community Health Workers at both the New London and Newport Primary Care Practices, collaborate with the Inpatient Care Managers to support patient psychosocial needs during hospitalization and as they relate to discharge planning, and respond to referrals from the Emergency Department and other outpatient departments.
Responsibilities
* Provides complete psychosocial assessments of identified patients and families.
* Provides emotional support to individuals and families facing social and emotional problems.
* Coordinates community resources to address/meet identified needs. Assists patients through application process for resources, as needed.
