$15,000 Sign on Bonus REPORTS TO DIRECTOR OF CLINICAL QUALITY BROAD FUNCTION: Responsible for planning the scope and carrying out the plan for the hospital social service program in accordance with the identified needs of the hospital and accepted social work practice. MAJOR RESPONSIBILITIES: 1. Use high risk screening criteria, conduct psychosocial assessments, develop, evaluate, implement and coordinate appropriate admission or discharge plans to meet the needs of the patient with full participation of the patient and family, as well as the goals of the hospital. Makes referrals to Discharge Planning, Nursing Staff and other appropriate agencies as deemed necessary. 2.
