Liquid Personnel is seeking a dedicated Social Worker to join its client’s Hospital Discharge Team in the London Borough of Richmond upon Thames. In this role, you will play a crucial part in ensuring safe, timely, and person-centred hospital discharges, helping adults return to their homes or community settings with the right care and support. This position promotes independence, wellbeing, and embraces a “Home First” approach.


What will your responsibilities be?


In this role, you will:

  • Coordinate and manage the discharge process for adults with diverse and complex needs.

  • Work within the Discharge to Assess (D2A) framework to facilitate timely transitions.
