Summary The GS- 11 Patient Aligned Care Team Social Worker independently provides clinical psychosocial services to eligible Veterans assigned to Primary Care Patient Aligned Care Teams. As a member of the interdisciplinary treatment team, the incumbent maintains knowledge of the PACT team approach and the role of each health and allied health care team member, as well as an understanding of the role of the social worker per local and/or national policy and directives. Responsibilities The duties include, but are not limited to: Independently assess the psychosocial functioning and needs of patients and their family members and formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
