Job description

The Central Wellbeing Team are currently looking for a highly motivated and dynamic individual to join us at Torfaen County Borough Council, Adult & Communities division. We are a multi‑disciplinary team that works together with partner agencies to meet the social care needs of adults from the age of 18. Our aim is to work proactively with individuals, to promote choice, control, and positive risk taking to achieve outcomes in areas that matter most to the individual.
