About the Role
To provide social work services to patients and their whanau of Te Whatu Ora- Southern by:
* Undertaking social worker assessments, interventions and evaluations
* Working with the MDT ensure safe care plans
* Protecting vulnerable patients from harm because of abuse and neglect
* Collaborate community service providers to deliver wrap around supports for patients

In this role you will be involved in:
* Undertaking accurate and comprehensive assessments and evaluations
* Plan and implement appropriate interventions
* Provide relevant education - including any relevant alternative options - in a format that can be clearly understood
