Job Description
Information & Referral
- Attend to walk-ins and telephone enquiries. Refer to the relevant staff for follow up if necessary.
- Conduct intake assessments.
- Provide appropriate information and refer clients to relevant agencies for further intervention.
- Manage intake administration.
- Manage assigned cases and case referrals.
- Conduct assessment, planning and intervention for cases.
- Develop and review appropriate intervention strategies.
- Participate in and/or conduct case conferences.
- Evaluate and document case outcomes.
- Complete case recordings and update client information and documentation in the case management system.
