Job Description
Service Coordinator
Provides coordination and monitoring of services for children with developmental delays, disabilities, or special needs, and supports their families in accessing appropriate care and resources.
Responsibilities
- Coordinate developmental assessment appointments and therapy services.
- Act as the primary liaison between families, healthcare providers, therapists, schools, and community agencies.
