Job Summary: In accordance with the organization’s mission, vision and values, the Residential Counselor is responsible for case management services. The following duties will apply:
Duties
+ Responsible for the development of a comprehensive evaluation, phone screens, development and implementation of individual client service plans, ensuring that plans are initiated; provide follow-up and modification as necessary. Assure that record keeping is in compliance with agency and OASAS regulations and policy & procedures.
+ Demonstrates teamwork and an ability to work in a team environment with coworkers and supervisors. Demonstrates the ability to be flexible and cooperative with the team.
