Job Description

The role

will also involve:
  • Liaising with families and health professionals

  • Attending multi-disciplinary (MDT) meetings

  • Creating care plans and risk assessments

  • Reviewing documentation, including quarterly reviews, to ensure care plans remain accurate and effective

    • Our service operates seven days a week, offering a programme of activities and therapies in conjunction with health professionals where appropriate. These are tailored to each individual’s level of need and are designed to:

  • Maintain and improve health and wellbeing

  • Promote independence and personal development

  • Provide valuable respite for families
