Job Description
**RESIDENT CARE COORDINATOR - FULL TIME - MONDAY-FRIDAY - 8:00am-4:30pm**
**JOB PURPOSE:**
Ensures the quality care of all residents. Works closely with the Administrator, Health Services Director, Dining Services Director, Life Enrichment Coordinator, Housekeeping and Environmental Services Director. This position requires creativity and flexibility with residents who have complex situations and is at times stressful. Must train resident aides in tasks required for delivering quality care to the residents of the community.
**KEY RESPONSIBILITIES:**
1. Coordinates care plan conferences with each resident and responsible family members, sending letters out on a monthly basis, and then following up with resident’s family members.
2. Maintains current tasks lists for resident aides, and update timely as ADL changes occur.
3. Schedules the service plan dates according to state regulation guidelines.
