**Job Summary and Responsibilities**
As our Rehab and Admission Discharge Social Worker, you will play a vital role in obtaining payor authorizations for rehabilitation admission.
Every day, you will perform social work assessments, interventions as needed, and planning for patients discharge needs. You will manage multiple priorities to create ethical, compassionate care plans.
To be successful in this role, you will must have excellent understanding of rehab admission criteria, both pre-admission, admission and discharge and insurance payor.
+ Creative, compassionate problem solving.
+ Effective collaboration with payors.
+ Timely documentation for admission authorization review.
+ Chart Review.
