In this part-time role, you will report to the Director of Health and Wellness while collaborating with the interdisciplinary team to ensure goal-driven care. Your responsibilities will include managing care plans, administering treatments, and ensuring compliance with quality standards.
Key Responsibilities:
• Coordinate resident care and documentation processes
• Evaluate and adapt care/service plans as needed
• Provide urgent and emergency care when required
• Facilitate patient and family education sessions
• Support care transitions for residents effectively
Requirements:
• Valid RPN registration in the province/state
• Experience in long-term or complex care settings
• Excellent communication and customer service skills
