Job Description
**BAYADA Home Health Care** is seeking a Per Diem **Registered Nurse (RN)** to join the Fort Collins Visits team and work with adult and senior clients on a per visit basis.
Please note: current or recent experience in a Medicare Certified Home Health setting with OASIS documentation experience is required for a per diem position.
As a home health nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.
**How You’ll Make an Impact:**
+ Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
+ Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
