Summary The Registered Nurse - Outpatient executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex outpatient care. This position is located at the La Crosse, WI Community Based Outpatient Clinic as part of the Tomah VA Medical Center. Responsibilities Assume responsibility for the coordination of care focused on patient transition through the continuum of patient care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction.
