Registered Nurse - (Home Based Primary Care)

Summary The VA Northeast Ohio Healthcare System is recruiting for Registered Nurse. The Registered Nurse will function within Home Based Primary Care section of the Community Outpatient Services. Responsibilities Completes independent home visits to Veterans with complex medical and psychosocial conditions. Provides direct nursing care and education to patients and caregivers based on the individualized plan of care initiated by the RN Case Manager. Collaborates with the HBPC Interdisciplinary Team which includes the Physician Preceptor, Registered Nurse, Licensed Practical Nurse, Occupational Therapist/Physical Therapist, Social Work, Dietitian, Speech Therapist, Psychologist, Pharmacist and Advanced Practice Provider. Collaborates with services internal and external to the VA to manage the care needs of the patients.
