Overview
Looking for a Registered Nurse to build relationships with clients and work with a dynamic, supportive team in a rural county? As the Registered Nurse Case Manager, you will direct the care of clients under the Home Care program, assess needs, coordinate interdisciplinary care, facilitate teaching, develop care plans, and play a key role in referrals. Your experience in wound care, wound care principles, products, palliative care, and home parenteral therapy will be essential. This role may include covering additional assignments as workload dictates.
