Job Overview

South Savo Wellbeing Services Eloisa is seeking a practical nurse for a permanent position at the Geriatric Assessment Department in Savonlinna, starting from 24 August 2026 or as agreed. Interviews will be conducted in weeks 33 and 34. The goal of the Geriatric Assessment Department is to thoroughly assess the overall condition of elderly patients, plan necessary rehabilitation measures, and develop a follow‑up care plan. The aim is always to discharge the patient to their home if it is feasible within their functional capacity.

Responsibilities

• Work in a three‑shift schedule as part of a multidisciplinary team.
• Provide comprehensive, individual‑centered care.
• Assess the patient’s functional ability and overall situation.
• Document all assessments carefully.
• Cooperate closely with nurses, physiotherapists, doctors, service guidance, gerontological social work, and housing services.
