Description
The Heart Transplant Nurse Coordinator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
Responsibilities:
Work closely with the physicians to assess patients in clinic including follow up on labs/biopsies and other test results.
Manage all aspects of patient care post heart transplant.
Coordinate referrals to other specialists as needed and insure that appropriate follow up communication is sent to outside referring physicians.
Maintain Toronto Immigrant Employment Data Initiative (TIEDI) forms as required and other necessary paperwork.
Work closely with the inpatient Heart Transplant Coordinators to coordinate the care across the continuum.
