About Us:
The Royal Flying Doctor Service South Eastern Section
(RFDSSE) delivers high quality, accessible healthcare to people living in rural
and remote communities. As the contracted provider for the Western NSW Primary
Health Network (PHN), RFDSSE delivers the Keeping Healthy Puura Manti Program
(KHPMP)—a time limited initiative designed to improve chronic disease outcomes
across the region.
The program integrates allied health and care coordination
services to improve Chronic Disease Management (CDM) outcomes for people across
the region. As part of this program, our team works collaboratively to support self
management, improve clinical outcomes, and enhance the wellbeing of high risk
and underserved populations.
