Job Description
Responsibilities
- Conduct thorough medication reviews to identify potential drug interactions and ensure patient safety.
- Advise healthcare professionals on the selection and dosage of medications based on patient‑specific factors.
- Provide medication counseling to patients, educating them on proper usage, side effects, and storage of their prescriptions.
- Monitor patient outcomes post‑medication initiation, adjusting therapies as needed to optimize efficacy and safety.
Qualifications
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- State licensure as a registered pharmacist.
- Minimum of 2 years of experience in a clinical pharmacy setting.
- Familiarity with electronic health records (EHR) and pharmacy management software.
