Schedule: have scheduled patients 1 day a week

Provide patient care in a skilled nursing facility by conducting rounds in their short-term units

Conduct documentation at home with provided iPad laptop within 48 hours of seeing patient

Collaborate with facility staff to enhance patient care

Ensure timely and accurate documentation

Requirements

Valid full license as either LPC or LMHC
