Setting:Home Based-virtually rare occasion going into home.
Call/Weekends:Rotates on call for 1 week a month.
Position Overview:
Henry Ford Healthis seeking an experienced, full-time Nurse Practitioner to join the Division of Hospice and Palliative Medicine. This role provides longitudinal palliative care for patients with serious illness, typically those with a prognosis of less than two years, with a manageable panel of approximately 200 patients. The NP will provide expert symptom management, advance care planning and goals-of-care discussions, coordinate transitions to hospice when appropriate, and collaborate with specialty providers while helping patients establish home-based primary care when needed.
