This full-time, permanent role reports to the Manager of Clinical Services and emphasizes collaboration with residents, families, and healthcare teams in developing and executing care plans. You will also lead educational and evidence-based initiatives while mentoring staff in gerontological practices.
Key Responsibilities:
• Provide comprehensive primary care to residents
• Develop and evaluate individualized care plans
• Lead staff mentorship and educational initiatives

Requirements:
• Registration with the College of Nurses in Extended Class
