About the Role
We are seeking an enthusiastic, passionate and skilled registered health professional to join our Health of Older People team at Disability Support Link. Although our primary office is based in Hamilton, an opening could be available in Matamata. This will be a varied and exciting role that will work alongside clients, families, local communities and inpatient settings so that the process of a needs assessment and service coordination reflects both community strengths and client needs. You will be working in partnership with older people who have disability needs, and younger people with long‑term health conditions, to achieve optimum health and well‑being through appropriate assessment, planning, intervention, evaluation and education.
