About the Role

The ACT team provides intensive and flexible support to Tangata Whaiora presenting with complexity. The multidisciplinary team collaborates across services to ensure wrap‑around support that meets the needs of Tangata Whaiora and their whanau. Caseloads are fluid and indicative of the complexity of care required at any time.

Working Arrangements

• 7 day a week service, inclusive of public holidays (08:00 – 16:30 hrs)
• Early evening on‑call Monday to Friday (16:30 – 20:30 hrs); on‑call clinicians may need to return to work for urgent client related business
• Self‑rostering to support work/life balance
