Job Summary
Reporting to the director, the incumbent will be responsible for providing support to participants in assisting them in achieving goals that will lead to successful community living. The incumbent will be responsible for assessing individual’s level of functioning and mental health, developing, documenting, and implementing of individual rehabilitation plans and individual intake interviews. The incumbent will be responsible for all aspects of assisting participants with housing, community integration and employment. This is a bilingual position in both official languages – English and French (Not required but preferred). This position provides Emergency On-Call rotation. The incumbent is required to work days, evenings and weekends, 7 days a week on a rotation schedule. Daily schedule may vary according to program needs.
