Job Description
The **Medication Reconciliation Lead** Pharmacy Technician at PipelineRx (now a part of Evernorth Health Services) supports Remote Pharmacy Services by collecting and documenting accurate patient medication histories, assisting pharmacists, and performing related clinical and administrative tasks.
**Shift:**
You will be required to work anywhere from 8a - 3a, EST, Mon - Fri.
**Key Responsibilities:**
+ Obtain, verify, and document accurate medication histories by communicating with patients, caregivers, pharmacies, providers, and other healthcare professionals.
+ Support pharmacists by escalating medication discrepancies or clinical concerns and ensuring documentation is completed in electronic medical records.
+ Assist leadership with daily workflow coordination, staffing coverage, team communication, onboarding, training, coaching, and operational support.
