Permanent part‑time position (not locum) to augment a family practice with a long‑term care (LTC) panel caring for frail elderly.

Responsibilities
- Assist one of the existing three LTCI practitioners with the resident caseload.
- Provide regular proactive visits and conduct on‑site medication reviews.
- Attend care conferences, participate in 24/7 on‑call, and complete documentation in the charts.
- Collaborate with an interdisciplinary care team (physicians, nurse practitioners, nurses, allied health, and care home leadership) to deliver holistic, resident‑centered care.
