Job Description
LaSante Health Center is seeking a compassionate and organized Housing Specialist to support patients experiencing housing instability, eviction risk, homelessness, rental arrears, and other housing-related challenges.
Responsibilities:
+ Assess patients’ housing needs and develop action plans.
+ Assist with affordable housing, supportive housing, NYCHA, Section 8, shelter, rental-assistance, and eviction-prevention applications.
+ Help patients gather documents and complete applications.
+ Coordinate with landlords, housing agencies, legal providers, shelters, and community organizations.
+ Document all patient interactions, referrals, outreach, and follow-up activities accurately and promptly.
+ Track applications, deadlines, and patient progress.
+ Work closely with therapists, case managers, and medical providers.
