Hospice LPN - Fairfield Hospice Gen

Provide intermittent skilled nursing care in accordance with the established plan of care under the direction of the assigned primary nurse/supervisor. Gives direct client care which includes changing dressings and catheters, administering prescribed medication and treatments, giving necessary injections, enemas, ostomy care, and pain management. Educates the client’s family in terms of how best to cope with the illness and end of life care. Carries out nursing assessment and care based on referral data, client/family interview, and clinical observation of patient health status, symptom management for hospice and palliative care patients. Records daily progress on charts of clients visited. Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases. Consults with physician regarding client needs and plan of care. Assists in evaluation of care given by paraprofessional staff.
