Habilitation Manager (I/DD services)

**Responsibilities for the Habilitation Manager:**

+ Act as internal case manager and advocate for clients in the Habilitation program within **Erie, PA** and surrounding areas.
+ Participate in Person-Centered Planning processes
+ Participate in an on-call rotation every 6 weeks, Monday-Monday.
+ Collaborate with the client, family, Case Managers, Habilitation Techs and other teams to implement and evaluate client care needs and individual goals identified in the Person Centered/Individual Support Plan
