The Band 4 Frailty Associate Practitioner works under supervision to support the Frailty Team with a variable caseload of complex patients within a multi-disciplinary team structure.

 There is access to clinical expertise and oversight to enable the optimal care of our patients. The delivery of a high quality, effective and compassionate care is essential to our organisation.

 The Frailty Practitioner will work in the Emergency Department, Acute Assessment Unit and inpatient wards. They will be expected to support the Older Person’s Rapid Access Clinic when required and cover the Team on opposing site when required. They will work alongside the qualified staff on the Frailty Intervention MDT to ensure every patient living with frailty presenting with an acute health or social care need receives the most appropriate care in the most appropriate setting in a timely manner.
