  • Physical Therapy (PT) will conduct evaluations and patient care to improve functional mobility, reduce fall risk, increase strength, endurance, and independence to make a meaningful impact by improving patients’ quality of life. PT will work collaboratively with the multi-disciplinary health care team and community resources to provide the best outcomes for patients.
  • This unique physical therapist role will cover patient care in multiple settings; including, home health, acute care, inpatient rehab, and swing bed.

    • Qualifications:
