Family Systems Navigator at Sheguiandah

Contribute to community wellness as a Family Systems Navigator at the Sheguiandah First Nation Health Centre. Focus on providing essential support, intervention, and prevention services for families involved with child protection.
This full-time role reports to the Director of Operations. The Family Systems Navigator will deliver short and long-term services, create individualized Care Plans, and collaborate with internal and external care programs. Your focus will be on supporting children, parents, and families while promoting community engagement and resilience.
Key Responsibilities:
• Provide direct support for families facing child protection issues
• Develop individualized Care Plans and implement intake processes
• Coordinate educational workshops and parenting programs
• Maintain community linkages for effective service delivery
