About you:

We are looking for an enthusiastic, proactive, and person-centred individual to join our Early Intervention and Prevention Team as an Early Intervention and Sustainment Officer.
You will be passionate about helping people, support stable housing and preventing homelessness through early intervention, practical support, and effective partnership work. You will have experience working with vulnerable individuals and be confident in assessing needs, identifying risks, and delivering tailored solutions that improve outcomes for customers.
The successful candidate will be able to undertake assessments both over the telephone and face-to-face, building positive relationships with customers while working collaboratively with housing providers, support agencies, health professionals, and other partner organizations.
