Diabetes Education Specialist — Empower Patients & Care Teams

M42 Ltd. is seeking a Diabetes Education Specialist to empower patients with diabetes through individualized care plans and education.

You will assess patient needs, develop management plans, educate on medications, glucose monitoring, diet and lifestyle, and coordinate with a multidisciplinary team to improve outcomes.

Based on JCI-aligned standards, you will uphold DOH/DHA guidelines and contribute to continuous education programs for staff and patients.
