Job summary
The Band 5 Development CHC Assessor is an entry-level practitioner role designed to build the skills, confidence, and knowledge needed to become a fully autonomous CHC Assessor.
This role forms part of a deliberate grow our own approach, giving developing clinicians the structured support, supervision, and learning opportunities they need to progress within the Continuing Healthcare pathway.
The postholder works under the guidance of experienced CHC Assessors and Senior Assessors, taking on aspects of the CHC assessment and review process in a gradual, supported way.
They contribute to evidence gathering, liaise with professionals, assist with completion of Decision Support Tools, and learn how to interpret the National Framework in real-world practice. As their competency develops, they will take responsibility for more complex tasks and increasingly work independently.
