Community Nursing (District Nursing) have teams based in the following areas;

·North Hub- Haxby Health Centre

·Selby/South Hub -Selby Hospital, Tadcaster Health Centre or Sherburn in Elmet

·East Hub- Tang Hall Clinic

·West Hub- Acomb Health Centre

·South Hambleton and North Ryedale- Easingwold, Pickering and Kirkbymoorside

We have a vacancy at Haxby Health Centre. 

We are community nursing teams comprising of motivated professionals, working out of the hospital setting delivering nursing care in patient’s homes and community settings. We have close working networks with our hospital, social and primary care colleagues to support patients to stay in their homes and avoid hospital admission.
