Community Clinical Needs Assessor — Home & Phone Assessments

Health New Zealand | Te Whatu Ora is seeking a Clinical Needs Assessor to screen, triage, and coordinate services for adults 16+ with age-related disabilities or chronic health conditions. Assessments may occur in home settings or over the phone, with occasional travel across the Northland region.

The role supports older people and those with high needs, and requires a current practising certificate in Nursing, OT, Social Work or Physiotherapy, plus strong communication and IT skills.
