Join our team as a Full-Time Clinical Pharmacist Specialist!
_This position will work primarily day shifts, with some evening rotations required, supporting the Transplant Team. This position requires availability to work every 4th weekend._
The Clinical Pharmacist Specialist provides specialized pharmaceutical care to pediatric transplant patients in both ambulatory and inpatient settings. Responsibilities include medication therapy management, patient and family education, multidisciplinary collaboration, and cross-coverage of transplant clinics and inpatient services to ensure continuity of care and optimal patient outcomes.
Primary Responsibilities:
+ Deliver direct patient care through real-time medication management within assigned transplant services with potential support of additional care areas and teams.
